Guardian Home Health Care 24-Hour Emergency Assistance

Guardian Home Health Care now offers a 3-tiered 24-hour emergency assistance service for individuals on the BI, CADI, CAC, and DD waivers. 24-Hour Emergency Assistance provides on-call counseling, crisis problem-solving, and immediate responses to health or personal emergencies. This service supports individuals living in the community and is designed to supplement—not replace—911.

Purpose

To deliver immediate support and crisis intervention, empowering individuals to live independently rather than in 24-hour facilities. This service is also ideal for those transitioning to independent living or in need of occasional on-demand assistance.

Eligibility

This service is available to individuals who qualify under specific home and community-based waivers—BI, CADI, CAC, or DD—and who require consistent or emergency in-home support.

Service Tiers

Tier 1 Services:

$40/day

Phone Maintenance

This tier provides remote support for urgent but non-life-threatening issues, without face-to-face contact.

Services include:

  • Triage and 911 assistance when appropriate
  • On-demand check-ins via phone or video
  • Coaching and crisis support aligned with the individual’s care plan
  • Response to technology alerts based on a customized plan
  • Coordination with the individual’s support team for follow-up training
  • Activation of the person’s emergency contact tree

Tier 2 Services:

$60/day

Phone + Limited In-Person Response

This level adds limited in-person support to scheduled and on-demand phone assistance. It’s designed to prevent crises and avoid escalation to emergency services.

Services include:

  • All Tier 1 services
  • Scheduled phone or video check-ins (e.g., to confirm provider arrival or individual’s return home)
  • Limited, warranted in-person responses
  • Emergency backup support if the primary caregiver or provider is unavailable
  • Physical staff response for urgent problem-solving and connection to resources
  • Blended approach with scheduled and unscheduled support

Tier 3 Services:

$80/day

Full Support + Weekly In-Person Visits

The most comprehensive tier, offering full 24/7 phone access and regular face-to-face interaction. Staff will respond physically to emergencies in the home or community. Additional check-ins are customized based on individual needs.

Services include:

  • All Tier 1 and Tier 2 services
  • Weekly in-person visits for support and problem resolution
  • Timely physical response to provide direct assistance and resource coordination
  • A personalized mix of scheduled contact and real-time emergency responses
